社居耆健醫學堂 – 計劃開展典禮

 

香港醫學會一向以「維護民康」為己任,積極推動醫學界參與社區服務,切實在社區體現「醫社合作」的概念。本會繼2016年獲「社區投資共享基金」(基金)撥款推行「社區健康醫學堂」計劃,得到各界的支持和成功後,2018年再次獲基金撥款,於葵涌區推行為期三年的「社居耆健醫學堂」計劃。計劃於二零一八年十一月至二零二一年十一月進行,並集結社、醫、福、校、房、商各界力量,透過推行:

 

(一)「老友記健康大學」:為區內長者、護老者及居民提供由醫生及專業人士主講的「身心健康課程」,以加強他們的自我健康管理能力。為物業管理公司、企業及區內小商戶員工舉辦「認識『認知障礙症』課程」,以強化社區支援網絡。

(二)「老友記外展大使隊」:組織醫生、護士、醫科、復康及護理學系學生、中學生及葵涌區居民成為外展大使,關顧葵芳邨及葵翠邨內有需要的長者及作定期家訪,識別及協助轉介有需要人士到合適的社區服務,發揮守望相助精神,建立鄰里互助網絡。

(三)「醫生單位一線通」:建立葵涌區內的醫生與長者服務單位的合作平台和轉介機制,並由醫生提供有關健康諮詢、轉介、講座和外展等服務,為區內長者及護老者提供適時的協助。

 

今天下午香港醫學會「社居耆健醫學堂」正式於葵芳邨開展。大會向參加者介紹計劃內容,在場設置攤位遊戲供參加者學習簡單的健康資訊,「老友記外展大使隊」的第一次探訪活動亦會於同日下午舉行。而「老友記健康大學」的身心健康課程將於2019年1月正式開課。

 

食物及衞生局局長陳肇始教授,JP,主持開展禮時表示:「香港醫學會得到勞工及福利局社區投資共享基金的撥款,在葵青區舉辦為期三年的『社居耆健醫學堂』計劃,透過醫、社、福等界別的合作,為長者提供學習和社交的平台,加強區內長者、護老者及居民對護老資源的認識。這計劃的理念,跟我們將來地區康健中心的工作可謂一脈相承。一個全面、協調各方設施、各界服務的基層醫療健康系統,能便捷地在社區內照顧市民大眾的個人需要,減少重覆入院及糾正以急症服務作為求診首個接觸點的現象。長遠而言,更可提升整體香港市民的健康。」陳局長感謝醫學會在推動醫社合作在社區的不遺餘力,並祝福計劃能順利推展。

 

社區投資共享基金委員會委員周奕希先生,BBS, JP,出席活動時表示對醫學會在葵涌區推行的計劃感到興奮,知道計劃將與各地區人士及團體合作,舉辦健康課程和定期探訪,以及建立醫社合作平台。周先生希望有更多長者及社區人士透過此計劃獲益,讓長者能在更舒適、健康的社區安享晚年。

 

香港醫學會會長何仲平醫生,MH, JP,在致辭時提到醫學會於兩年前在柴灣區推行「社區健康醫學堂」時獲得相當熱烈的反應。今次再度獲基金撥款推行「社居耆健醫學堂」亦希望會在醫社合作上作更多的嘗試。何醫生亦感謝「社居耆健醫學堂」計劃小組委員會及九龍西社區網絡的一班熱心服務的醫生,在籌辦活動所付出的心血。

“Community Health Academy for Elderly” - Kick-off Ceremony

 

In furtherance of the Hong Kong Medical Association’s motto, “To Safeguard the Health of the People”, and to drive the medical sector into active participation in community services, so as to fortify the concept of “Medical-Social Collaboration in the Community”, the HKMA applied funding from the Community Investment and Inclusion Fund (CIIF) to run the “Community Health Academy” in Chai Wan in 2016, in which positive responses were received.  In 2018, the HKMA received a second funding support from CIIF to launch the “Community Health Academy for Elderly” project (the Project) from November 2018 to November 2021 in Kwai Chung district.  As in the project in Chai Wan, we adopt the model of multi-dimensional collaboration by connecting social, medical, community, school, housing and commercial sectors.  The Project consists of 3 main areas:

 

(A)  Health Academy for the Elderly: Targeting the elderly, carers and residents in Kwai Chung district, the “Wellness Course” will be delivered by doctors and professionals of related fields to help strengthen the participants’ health management ability.  “Get to Know Dementia Course” will be provided for property management especially security staff and door guards, and staff of corporates and small merchants, to enhance the support in the community. 

(B)  Outreach Team for the Elderly: To mobilize medical doctors, nurses, medical and other allied health students, secondary school students and residents in Kwai Chung district to become Outreach Ambassadors, to provide regular home visits to the singleton elderly or elderly couples, and help identifying those in-need.  To encourage mutual help and build up a supportive network.

(C)  Doctors-Units Instant Link Scheme: Building up a cooperative and referral platform between medical doctors and social service units in Kwai Chung district, so that regular health-related services, e.g. health consultation, timely referrals, health talks and outreach services can be arranged for elderly and carers in the community.

 

The Project was officially kicked-off in Kwai Fong Estate this afternoon.  The participants were introduced and briefed with the Project content and had taken part in the game booths to learn about disease management.  The “Outreach Team for the Elderly” also conducted their first home visit in Kwai Fong Estate on the kick-off day to lay down an important milestone.  Moreover, the first session of “Wellness Course” of the Health Academy will be held in January 2019.

 

During the kick-off ceremony, Prof. Sophia CHAN, JP, Secretary for Food and Health, remarked, “The Hong Kong Medical Association was granted funding from the Community Investment and Inclusion Fund under the Labour and Welfare Bureau to implement the 3-year project ‘Community Health Academy for Elderly’ in Kwai Tsing district.  Through the collaboration of various sectors, such as medical, social and NGO, a platform of learning and social network for elderly, carers and residents will be built to enrich their knowledge in elderly care resources.  Idea of this project is in the same vein of the District Health Centre (DHC) under the government planning.  There will be a comprehensive health system to coordinate different service providers and provide primary care service for the public on disease prevention and health management, so as to address the issues of revolving door of hospital admission and using A & E as the first point for health need.  In the long run, it is hoped that the DHC will help elevating health and wellness of the public in Hong Kong.”  Prof. CHAN thanked the HKMA in sparing no effort to promote the “Medical-Social Collaboration in the Community” through the Project and wished the Project every success!   

 

Mr. CHOW Yick Hay, BBS, JP, Member of the CIIF Committee, congratulated to the Project team.  He noticed that the “Community Health Academy for Elderly” would mobilize many medical doctors and medical students to collaborate with various social services units, schools and local communities in organizing health education programmes and home visits on a regular basis.  Mr. CHOW hoped that more elders and residents would be benefited from the Project, by enjoying a more comfortable and healthy living environment in the community.

 

The HKMA President, Dr. HO Chung Ping, MH, JP, recalled that the success of the “Community Health Academy” project in Chai Wan which was just completed in 2018.  The new Project in Kwai Chung districts would explore more potential collaborations with social and community.  Dr. HO also thanked the colleagues of the Sub-Committee on Community Health Academy for Elderly and members practising in the Kowloon West districts on driving the programmes.

Community Investment and Inclusion Fund (CIIF) was set up by the Government of the Hong Kong Special Administrative Region in 2002 to implement diversified social capital development projects in the community, promote reciprocity between the public and different sectors, and build together a cross-sectoral collaborative platforms and social support networks.  The Fund seeks to build social capital – to garner mutual trust, spirit of cooperation and social cohesion, and enhance mutual support among individuals, families and organisations so that our community can grow from strength to strength.
