老友記健康大學結業禮暨老友記外展大使嘉許禮

 

香港醫學會於二零一六年獲社區投資共享基金撥款,在港島柴灣區開展為期兩年的「社區健康醫學堂」計劃,目的是貫徹香港醫學會「維護民康」的宗旨及推動醫學界積極參與社區服務,切實在社區體現「醫社合作」的精神。計劃包括三大部分-「老友記健康大學」、「老友記外展大使隊」和「一服務單位一醫生」,旨在強化長者及護老者的支援網絡,優化區內的長者健康服務。兩年的計劃將於本月底結束。

 


今天下午香港醫學會特別舉行「老友記健康大學結業禮暨老友記外展大使嘉許禮」,分別頒發畢業證書及義工嘉許狀予一班好學的老友記學員和熱心服務的外展大使。近200位嘉賓及參加者出席,一同見證計劃的成果,場面熱鬧。

 

一眾主禮嘉賓首先致詞為活動揭開序幕。香港醫學會會長何仲平醫生,MH, JP在致歡迎詞時公佈醫學會將於11月分別開展兩個社區服務計劃。其一是由社區投資共享基金撥款並於葵涌區推行的三年計劃「社居耆健醫學堂」。另一個則是由醫學會自行撥款在柴灣推行一年的「社區健康醫學堂」延伸計劃。何醫生特別感謝每一位為計劃付出過努力的人士,及與計劃協作的團隊,成就出一個真正連結醫社界别的計劃!

 

勞工及福利局副局長徐英偉先生,JP在致詞時表示特區政府已為《安老服務計劃方案》制訂四大策略方針,當中加強社區照顧服務及減少住院比率一項與「社區健康醫學堂」所推動的服務目標非常吻合。徐副局長亦讚揚醫學會的計劃帶動年長的市民更關注個人和家人的健康,同時連繫社區上的社福機構,在社區上為區內市民建立健康支援和關顧網絡,也為長者居家安老作好準備。

 

而社區投資共享基金委員會主席暨安老事務委員會主席林正財醫生,BBS, JP亦分享自己由兩年前見證「社區健康醫學堂」計劃的誕生到現在即將完滿結束,得知計劃為柴灣區鞏固了健康和互信互助的社會資本,讓老友記和市民更懂得關顧自己和身邊人,實在是非常感動! 林醫生亦冀盼醫學會即將在葵涌區推行的新計劃能為該區建立穩固的社區健康支援網絡,他又感謝醫學會在推動社會資本和社區健康資訊方面不遺餘力。

 

典禮上,徐英偉副局長、林正財醫生、何仲平醫生、聯同社會福利署東區及灣仔區福利專員葉巧瑜女士、香港醫學會義務秘書暨社區服務委員會聯席主席楊協和醫生、三位香港醫學會會董暨社區服務委員會聯席主席陳念德醫生、龐朝輝醫生及唐繼昇醫生、以及兩位參與計劃的老友記學員劉玳汝女士和老友記外展大使林月詹女士,一同主持「醫社互緊扣」儀式,象徵著社區上的有心團隊共同參與社區服務,並將社會資本緊扣起來,令地區服務能夠百花齊放、薪火相傳。而兩位計劃參加者亦即場分享參與計劃的點滴。

 

嘉賓們其後頒發畢業證書予55位修畢指定學分的「老友記健康大學」學員,及分別獲得金銀銅嘉許獎狀的「老友記外展大使」(分別為金章:18位、銀章10位、銅章:16位)。嘉賓同時向被訪長者致送由外展大使為被訪長者製作的《老友記一步一印記》人生故事。表演環節由社區服務委員會聯席主席帶領一班老友記學員、外展大使,以及被訪長者高唱《醫社一心》一曲,激起全場熾熱氣氛。

 

「社區健康醫學堂」計劃為市民製作了一系列的健康資源,包括:老友記健康大學《實用手冊》和《常見老年疾病動畫短片》。所有資源已上載到醫學會網頁/專頁(詳見《補充資料》),另亦於溫莎公爵社會服務大廈地下大堂電視播放。市民可到相關連結了解更多詳情。

Graduation Ceremony of Health Academy for the Elderly cum

Recognition Ceremony of Outreach Ambassadors for the Elderly

 

The Hong Kong Medical Association (HKMA) was funded by the Community Investment and Inclusion Fund (CIIF) in 2016 to run a 2-year “Community Health Academy” project in Chai Wan district, with the objectives to actualize the Association’s motto, “To Safeguard the Health of the People”, and to drive the medical sector in active participation in community services, so as to strengthen the concept of “Medical-Social Collaboration in the Community”.  The Project consists of 3 main areas: “Health Academy for the Elderly”, “Outreach Team for the Elderly” and “One Service Unit One Doctor”, aiming to strengthen the support network for the elders and their carers with a view to improving health care services for the elderly in the district.  Programmes of this 2-year Project would be completed by end of this month.

 

“Graduation Ceremony of Health Academy for the Elderly cum Recognition Ceremony of Outreach Ambassadors for the Elderly” was held in this afternoon to show appreciation to the Project’s active participants.  Almost 200 guests and participants joined the event and witnessed the achievement of the Project.

 

In his welcoming remarks, the HKMA’s President Dr. HO Chung Ping, MH, JP, announced that HKMA would carry out two new community projects in November 2018.  One would be another CIIF funded project named “Community Health Academy for Elderly” which is a 3-year project to be started in Kwai Chung.  Another one would be a 1-year additional project of the “Community Health Academy” in Chai Wan which is self-financed by HKMA.  Dr. HO expressed his heartfelt thanks to every participant especially those collaborating partners of the Project.  Without their unceasing supports, the Project could not be as successful as what it is now.    

 

Mr. TSUI Ying Wai, Caspar, JP, Under Secretary for Labour & Welfare, denoted that according to the four strategic directions confirmed by the Government in its Elderly Services Programme Plan (ESPP), the objectives of “Community Health Academy” indeed are in line with one of the strategic directions on achieving “ageing in place” and reduce institutionalization rate through more community care services.  Mr. TSUI also praised the Project by raising health consciousness among elderly residents, and strengthening the connection with local NGOs by setting up different networks and platforms, in enhancing the readiness of the community for “ageing in place”.        

 

Being a witness of development of the Community Health Academy, Dr. LAM Ching Choi, BBS, JP, Chairman of CIIF Committee cum Chairman of Elderly Commission learnt that the Project had well promoted health awareness and consolidated mutual trust and support among participants in Chai Wan, and as a result more elders and local residents become aware of their own health conditions and reach out to those in need.  Dr. LAM hoped that HKMA, by initiating a new CIIF project in Kwai Chung, would also build up a strong support network in that community.  He also thanked HKMA for sparing no effort in promoting social capitals and health education in the community. 


 

 

Mr. Caspar TSUI, Dr. LAM Ching Choi, Dr. HO Chung Ping, together with Ms. YIP Hau Yu, Hannah, District Social Welfare Officer (Eastern & Wan Chai), Dr. YEUNG Hip Wo, Victor, HKMA Honorary Secretary cum Co-Chairman of the Community Service Committee, 3 other HKMA Council Members cum Co-Chairmen of the Community Service Committee, Dr. CHAN Nim Tak, Douglas, Dr. PONG Chiu Fai, Jeffrey, Dr. TONG Kai Sing, Ms. Daisy LAU, elderly student of the Academy and Ms. LAM Yuet Chim, an active outreach ambassador, officiated the “Bridging up Medical-Social Sectors” ceremony.  The well-planned ritual symbolized enthusiastic teams and individuals worked hand-in-hand in the community to build social capitals, which generated a variety of meaningful services in the district.  After the ceremony, two Project participants shared their experiences in the Project. 

 

Fifty-five eligible elderly students who have attained the required study credits were awarded with the Graduation Certificates.  In addition, outreach ambassadors were presented with the Certificates (18 gold awards, 10 silver awards and 16 bronze awards) to recognize their outstanding services.  Some groups of outreach ambassadors have made “Life Storybooks” for the elderly households they visited, the finished storybooks were presented to the elders during the Ceremony.  Outreach ambassadors along with the elderly they visited and the elderly students, led by the 4 Co-chairmen of the Community Service Committee, performed a song “Medical-Social as One Heart”, lyrics of which were specially written for the occasion.   

 

The Community Health Academy has produced a series of health resources, including the Practical Handbook of the Health Academy for the Elderly and the Animated Videos on Common Elderly Diseases.  The information has been uploaded to various electronic platforms (please refer to “Supplementary Information”).  The videos were also being broadcasted in the lobby of the Duke of Windsor Social Service Building.  The general public can check out the various channels for the related resources.

Community Investment and Inclusion Fund (CIIF) was set up by the Government of the Hong Kong Special Administrative Region in 2002 to implement diversified social capital development projects in the community, promote reciprocity between the public and different sectors, and build together a cross-sectoral collaborative platforms and social support networks.  The Fund seeks to build social capital – to garner mutual trust, spirit of cooperation and social cohesion, and enhance mutual support among individuals, families and organisations so that our community can grow from strength to strength.
